Case Management
Great Bay Services is licensed by DHHS to provide MaineCare-funded Case Management services to adults age 18+ with a diagnosis of an Autism Spectrum Disorder or Intellectual Disability.

Our experienced and knowledgeable Case Managers help identify personal goals, strengths and support needs to maximize independence. We assist individuals in exploring and accessing resources that help them develop a sense of well-being, purpose, and belonging in the community.
Our Case Managers work as dedicated advocates that care tremendously about securing the services your loved ones need. With small caseloads and low turnover at our facility, we really get to know and care for your family in ways other agencies cannot. Service Area: Southern Maine |

Community Supports
Great Bay Services offers a wide selection of educational programs, trips and events in the areas surrounding our Maine and New Hampshire locations, allowing each individual we serve to revel in each new skill they learn, each new activity they participate in, and each new opportunity to grow.

Participants enjoy outings including but not limited to; local libraries, the YMCA, bowling, swimming, wildlife refuge's/estuaries, hikes, sporting events, and amusement parks. Our individuals also enjoy trivia and a variety of cooking, music, art, drama, and gardening classes. Our Outdoor Discovery program is made up of experiential exploration, environmental education, and community-based volunteering and learning. These activities include but are not limited to hiking, fishing, snowshoeing,trail maintenance, archery, nature study, and Geocaching. |
Volunteerism
It feels good to give back! We support and encourage volunteer efforts with local non-profits including Meals on Wheels, animal shelters, local libraries, senior centers, soup kitchens, and other venues that seek volunteers.

Employment Supports
Work Supports
Work supports are based on the needs of the Job Seeker. Some Job Seekers require extra help doing their job. Our Job Coaches provide that support. They work with the Job Seeker providing the support, encouragement and if needed helping them to complete the parts of the job they may struggle with. The Job Coach is not there to do the job for the Job Seeker but to teach, support, direct and guide. The Job coach will help to create a natural support team and work with the employer to help build the independence of the Job Seeker.

Example:
The Job Seeker may need help setting up their workstation but can do the rest of the job without support. The Job Coach would help with that piece and leave for the rest of shift. The Job Seeker may need support during the entire shift. Then the Job Coach would stay with the Job Seeker working alongside them by guiding, directing, teaching, and supporting. Sometimes the Job Seeker needs help to navigate online trainings, the Job Coach would help by teaching and even reading and explaining the on line course. |
Employment Specialist Check-In
When the Job Seeker can do the entire job and works with the natural supports around them. The Employer would still like to have contact with agency to help them communicate the Employers needs or Employee’s needs, to make sure employee is happy, understanding his or her job expatiations, help if struggling, come back in to help the Job Seeker learn a new task, give employer feedback, and vice versa or even help with evaluations, paperwork, and online trainings...etc...
Career Planning
Helping potential employees to find their employment path. Some may have a job already but would like to change careers. Career Planning gives the person the support needed to do so. The Career Planner works with the focus person and the people who know them best. They focus on the persons interest, strengths, surroundings, hobbies, goals. It’s about getting to know the person and exploring the surrounding community businesses. Setting up tours and work trials based on interest. We are not looking for a job but exploring the different businesses so the person can get a feel for what is out there. The Career Planner comes up with themes based on the persons interest and they use these themes to find businesses with some or all the themes. This report is compiled and then shared with Vocational Rehabilitation Services.

Independent Living Supports
Great Bay Services' Independent Living Program is designed to serve members who are currently receiving Section 21 and Section 29 waivers. The mission of Independent Living is to help maintain and strengthen independence for individuals who are either currently living at home with their families or roommates as well as to recipients who live in their own homes or apartments.

Housekeeping, laundry, ensuring safety in the home such as monitoring smoke alarms, potential hazards for falling, expired foods etc. Health and nutrition support to include menu planning, grocery shopping, price comparisons, storing foods safely, meal prep, cooking, and portion control. Budgeting and banking, bill paying, saving for incidentals and procuring and securing entitlements. Personal hygiene supports to include cues, reminders or physical assistance with showering, proper dental care, appropriate weather attire etc. Education and modeling to learn public transportation when available. |
Medication oversight of accuracy, follow through, refills, reporting missed dosages', monitoring side effects, assistance with administration of medication by certified CRMA’s. Support with scheduling, transportation, and advocacy to medical, dental, and psychiatric appointments. Self-management in the areas of safe and responsible behavior, advocating in the members’ best interest including conveying information and interpreting information. This also includes teaching coping skills, giving emotional support and assistance with accessing any necessary resources needed.
Monitoring and reporting of a member’s personal wellbeing specifically in the areas of possible abuse, neglect, and exploitation. Encouragement and assistance with accessing community events and social opportunities.
All services are designed to support each individual member reach his/her personal goals as outlined in their personal care plan.
Independent Living offers flexible hours including evenings and weekends seven days a week to people who work and/or attend other programs.
